A heat pump is an air conditioner that can run backwards, so it has everything an AC has plus two parts an AC does not: a reversing valve and a defrost cycle. Most heat pump faults are one of those two, or the same refrigerant and airflow issues any cooling system has.
Cool air in heating mode. Usually the reversing valve, the component that flips the refrigerant flow between heating and cooling. When it sticks partway the system runs but delivers the wrong temperature.
The outdoor unit is iced over and stays that way. Some frost in winter is normal and the defrost cycle clears it. Ice that never clears means the defrost control, the sensor or the reversing valve is not doing its part, and running through it strains the compressor.
Running constantly without reaching temperature. Refrigerant leaks are the usual cause. It can also be normal: below about 35 degrees an air-source heat pump works harder for less, which is what the backup heat strips exist for.
Enormous winter bills. Often the heat strips running when they should not, because the thermostat or a sensor is telling the system it needs emergency heat. Resistance heat costs several times what the heat pump costs to run, so this is worth chasing down.
The system will not switch modes at all. Reversing valve or thermostat wiring. Worth troubleshooting properly, because replacing a thermostat that was never the problem is a common waste.
Repair cost tracks the part, and heat pumps have a wider spread than most equipment. A thermostat, a defrost sensor or a capacitor is at the low end. A reversing valve is labor-heavy because the refrigerant has to come out and go back in. A compressor is at the top and is usually the point where replacement enters the conversation.
Refrigerant is the variable that catches people out. Finding and fixing a leak is a real repair; recharging without finding it is not, and on an older unit the refrigerant itself can cost more than the labor.

Should the outdoor unit run in winter? Yes. A heat pump pulls heat from outside air even in the cold, which is why the outdoor unit runs year round. That is normal, and it is the difference between a heat pump and an air conditioner that sits idle from October.
What is auxiliary or emergency heat? Electric heat strips that assist when it is too cold for the heat pump alone. Occasional use is by design. Constant use means something is wrong and your bill will show it.
Do heat pumps work in Missouri winters? Yes, and modern ones considerably better than the reputation suggests. They lose efficiency in a hard freeze, which is what the backup heat covers, and they do the rest of the season for less than a furnace.
